Apple Watch Ultra 4 could receive a complete redesign and a new blood pressure monitoring feature, according to MacRumors.
According to the report, Apple is developing a high blood pressure notification feature that would use the optical heart-rate sensor on the back of the watch.
The system would analyze how blood vessels respond to each heartbeat and send alerts if abnormal patterns are detected. The feature is reportedly currently under FDA review in the United States.
The article notes that earlier Apple Watch models already had some blood-pressure sensing capabilities, but the new version could offer a more refined or clinically validated implementation of the technology.
DigiTimes also reports that the Apple Watch Ultra 4 could receive a “full redesign” alongside a significant upgrade to its sensor system.
According to supply chain sources, the back of the device may feature eight sensors arranged in a ring pattern.
Analysts believe the updated design and new health-monitoring capabilities could increase Apple Watch sales by 20–30% compared to 2025.
Following the rollout of blood pressure monitoring, Apple is also expected to continue developing noninvasive blood-glucose monitoring technology, which the company has reportedly been working on for years.
Apple Watch Ultra 4 is expected to be unveiled in fall 2026 alongside the Apple Watch Series 12, iPhone 18 Pro, iPhone 18 Pro Max, and a foldable “iPhone Ultra.”
